SUPERVISED BY:  Assistant Director of Clinical Services                       




SUMMARY:  Responsible for screening and scheduling all intake and assessment appointments for the agency. Provide diagnostic assessment support to the Assistant Director of Clinical Services.



  1. Responsible for screening and scheduling all intake appointments. 
  2. Responsible for calling intake clients to confirm appointment times.
  3. Responsible for backfilling intake appointments.
  4. Greet clients upon arrival for assessment.  When indicated, assist clients with storing belongings in locked storage bin.
  5. Responsible for coordinating information with the intake team for all incoming clients.
  6. Responsible for working with the client to develop an Individual Treatment Plan according to OMHAS/CARF person/family-centered standards.
  7. Responsible for providing assessments according to OMHAS standards.
  8. Responsible for attending scheduled individual supervision sessions with the Assistant Director of Clinical Services.
  9. Responsible for conducting intake assessments, completing all necessary paperwork and utilizing the Diagnostic Assessment Form to assess the appropriate level of care based on the admission criteria according to OMAS standards.
  10. Responsible for providing mental health and substance abuse diagnoses based on DSM 5 and incorporate clinical impressions into a comprehensive treatment plan that includes the Ohio Outcomes identification of problem and functioning as part of their formulation.
  11. Responsible for completing required paperwork within 24 hours.
  12. Complete state waitlist information.
  13. Responsible for maintaining client attendance spreadsheet.
  14. Attend huddle call with GSH, when indicated.
  15. Utilize literature, workshops and other training aids to continually improve clinical knowledge and administrative skills.
  16. Attend clinical meetings, as needed.
  17. Work to attain the goals as written in job performance evaluations
  18. Abide by the standards as set forth in First Step Home’s Compliance Plan and Code of Conduct.
  19. Responsible for following all safety rules and safe health practices of First Step Home, Inc.
  20. Comply with all agency policies, procedures and directives of immediate supervisor, the President/CEO and the Board of Trustees of First Step Home, Inc.
  21. Responsible for following the Core Values of First Step Home, Inc.
  22. Other duties as assigned


  1. Bachelor’s Degree in Social Work required.
  2. LSW required.
  3. Three to five years’ experience with mental health and addiction counseling of adults preferred.
  4. CDCA required or must obtain within 90 days of employment 
  5. Strong computer skills.
  6. Strong communication skills.
